Cheshire and Merseyside Cancer Alliance has worked with primary care colleagues and tumour-specific Clinical Quality Groups to update the Urgent Suspected Cancer (2 week wait) referral forms. These will replace existing 2WW forms.

The forms reflect NICE guidance with the exception of the Lower Gastrointestinal form which follows updated guidance with the inclusion of FIT.

The forms request additional clinical information to help our patients get to the right clinic or straight to test as soon as possible which can save valuable time and worry for our patients. There are questions about any additional needs or reasonable adjustments to help our patients. If you wish to provide feedback, please email: ccf-tr.cmcaprimarycareengagement@nhs.net.

These forms are rolled out with the support of IT providers in each Place and will be available in your EMIS file structure. You will see below a copy of each referral form. You can open and view the content and ensure you are using the latest version. They cannot be imported into EMIS systems.

Speaking to patients to ensure they understand the reason for their referral is key. It is also helpful to have a letter to re-enforce this. Your Practice may already use one that works well. If not, this letter can be imported for use in your Practice to give to patients when they are being referred on an urgent suspected cancer (2ww) pathway. This information prepares patients for the fact they are referred to a “cancer” clinic, yet offers some reassurance and also emphasises the importance of being available for their appointment even if short notice.
