Please find below information on the Consultation Templates with Cancer Guidance which have been developed for EMIS by Cheshire and Merseyside Cancer Alliance by local GP’s Dr Cathy Hubbert and Dr Octavia Stevens working with Informatics Merseyside.
The Guidance Templates are designed to help Primary Care clinicians get the right patient to the right test or clinic as soon as possible, without extra work. They will prompt the clinician to do the necessary tests and examinations and offer the most up to date referral form, most of which will be completed from the consultation.
They include NG12 and local Cheshire and Merseyside pathway guidance and support decision making. They also include safety netting for referrals and tests and provide other important tumour-specific information.
Informatics Merseyside have provided demonstration videos and upload guides to support their use. The Guidance Templates are available with the support of IT Providers in each Place and will be available within your EMIS structure. To access them in your practice system, please follow the instructions below in the Training and Guidance drop down.
If you have any problems accessing the Templates, please contact your IT Service Provider listed below.
We really want your feedback for future developments of the Templates. You can let us know by emailing ccf-tr.cmcaprimarycareengagement@nhs.net

Below you can see two video case studies - Dr Cathy Hubbert demonstrates an example of how to use the Consultation Templates with Cancer Guidance when a patient presents with a “change in bowel habit”.
Part one guides the user through the Lower GI Template during the consultation, showing how there is instant access to all clinical decision support information. It also highlights how the Templates include the option to use the latest Cheshire and Merseyside Cancer Alliance Safety Netting tool.
Part two picks up the pathway once the patient’s abnormal FIT has been received and illustrates how the user can easily complete the urgent suspected cancer referral to secondary care.
Below you can find a flyer, if you require this in an accessible format, please contact: ccf-tr.cmcaprimarycareengagement@nhs.net
